CONFIRMATION  - A booking will only be confirmed upon receipt by Tribe Safari of a completed and signed copy of this form from a designated tour leader together with a 20 % deposit of the total amount quoted, constituting a 20 % deposit per person indicated on the booking form. The name of each participant of the tour must appear in the appropriate space designated for additional names.
LIABILITY - Tribe Safari and its agents accept no liability for any damage incurred through injury, death or loss of property by any member of the group indicated on this booking form through his/her participation in the tour. Travel delays, flight cancellations or changes to itineraries deemed necessary by Tribe Safari may occur from time to time. Tribe Safari and its agents accept no liability for any damage resulting from these or any other occurences deemed to have been beyond the control of Tribe Safari and its agents. By undertaking this package, you will/could participate in dangerous activities such as scuba diving or snorkelling etc. Any injury, loss or damage resulting from any such activity, but not limited to those mentioned here, will be entirely at your own risk and no liability whatsoever will be accepted by Tribe Safari or its agents. By signing this form, you are deemed to have agreed with and accepted these conditions as binding and indemnify Tribe Safari and its agents for any such loss or damage incurred on behalf of each participant of the tour indicated on this form.
CANCELLATION  - Cancellation of a booking or failure to join a tour will result in a retention of funds by Tribe Safari as follows : Upon receipt of written cancellation by any member of the tour indicated on this booking form 30 days or longer prior to departure, the 20 % confirmation deposit (of the cancelling party) will be retained by Tribe Safari and the balance of the package price refunded. If a cancellation is received at least 14 days and up to 29 days prior to departure, the 20 % deposit plus 50 % of the remainder of the package price will be retained by Tribe Safari. If a cancellation is received less than 14 days prior to departure, 100 % of the total package price will be retained by Tribe Safari and all outstanding payments will become payable immediately. Should the cancelling party be able to find a replacement for himself no less than 14 days prior to departure and the replacement completes this booking form individually, no retention of funds will take place. However, should no replacement be found by 14 days prior to departure, a 100 % cancellation penalty will apply to the cancelling party Tribe Safari and its agents reserve the right to terminate, at their discretion and with immediate effect, the participation of any person on the tour.
GENERAL - Please note that the total cost related to ground arrangements in a foreign country is deductable from the foreign exchange allowance for South African citizens and must be endorsed by the passport holder to the correct amount. A label, provided by Tribe Safari must be completed correctly and affixed in the appropriate place in the passport prior to departure.

ADDITIONAL INFORMATION :
FLIGHT TICKETS -
Upon receipt of full payment by each participant of the tour, flight tickets will be available for collection at the offices of Tribe Safari four weeks prior to departure. Flight tickets, once issued can not be changed in any way without the consent of Tribe Safari or the airline.
VISAS - Residents from most countries require visas to enter Tanzania. Please check whether your country is exempt from any visa requirements. South African residents require visas to enter Tanzania. Tribe Safari will arrange your visa for a nominal fee. In order for us to process your visa, we require a valid passport (valid for 6 months after departure) with two blank pages. Together with the passport, we require two passport photographs and a completed visa application form.
HEALTH - Tanzania is considered a Malaria risk area. Before visiting the country you should familiarise yourself with the disease and consult a specialist/physician for advice on the disease. Malaria can be fatal and should be taken most seriously. You are further required by the Tanzanian authorities to have a valid Yellow Fever inoculation prior to your entry into the country. The inoculation must be done at least 10 days prior to departure and you must be able to produce the inoculation certificate upon entering Tanzania. You are also advised to familiarise yourself with the prevention of other tropical diseases.

FINAL NOTE -  Should you require any medication whatsoever, you must provide your own and be able to administer such medication yourself. Medical supplies in Tanzania are not reliable or guaranteed.
